Minutes of the Quarterly Health and Safety Meeting held in Orkney 13th December 2010 at 10.00 am

The Chairman welcomed the delegates. 

  • The minutes of the previous meeting were read and approved. 

Regional Reports 
Anglo Platinum Rustenburg 
No safety incidents have occurred at Anglo Platinum operations during the last period.
Dempsey reported to the meeting that he needed an Index to the system. The request betrayed a lack of knowledge of the system and further training was scheduled for Friday 17th December 2010. 

  • Aquarius 

No incidents or accidents were reported.  
Randfontein 

  • Doornkop 

No Incidents or accidents occurred. 

  • Burnstone Mine. 

No incidents or accidents were reported. 

  • Palabora Mining Company 

No incidents or accidents were reported.
Pto`s. 
Total of 358 Pto`s done of which 30 failed and was re-done on Pulling of rods, Production drilling and Rigging of machine.

 QUARTER 4

Free State 

Welkom

·   Tshepong 

This contract continued to be the worst operation from a safety perspective. After 4 accidents during the last quarter this quarter witnessed an accident where a piece of core descended down the rod string and struck the injured on the foreman. The incident did not result in a lost shift.

  • Masimong

No incidents or accidents were reported. 

  • Joel

No incidents or accidents were reported.  

  • Phakisa

V Nqoko lacerated his finger whilst lowering rods as a result of a communication failure between himself and the operator. The incident was not serious and no shifts were lost. 

·    Target, Unicel, Merriespruit and Steyn Shafts. No incidents or accidents were reported. 

  •  Pto`s 

Total of 297 done with no failed Pto’s.

  

Vaal River  

 

·        Moab 

This contract continued to record no incidents or accidents for the period under review 

·        Kopanang Mine

No incidents or accidents were reported.  The platform design was reviewed by the mine and Lesedi personnel and some modifications were considered important before operationalising the concept. 

·        Gt Noligwa.

This contract continued to record no incidents or accidents for the period under review 

Two Rivers

No incidents or accidents were reported.  There are 4 machines drilling currently. 

PTO`S 

Total of 668 Pto`s done of which 12 failed and was re-done on face clamp operations and pulling of rods.

 

 

General

 

  1. Mr. Gerhard Victor the new Health and Safety Manager was welcomed to the meeting. 
  1. OSHAS 9001. John Phyfer proposed the plan for the audit of Rustenburg operations and Vaal River operations. January 10th will see the audit by Dolf De Noon of Townlands and Turffontein operations. On Thursday 13th January Vaal River operations will be audited by Dempsey Fouche. 
  1. A training session for Vaal River supervisors and managers will take place on the 21st December. 
  1. A training session for Graham Van Den Heever and Dempsey Fouche will take place at John Phyfer’s offices on Friday 17th December. 
  1. A legal compliance audit needs to be arranged. John Phyfer to contact consultants who will conduct this audit. 
  1. The following Risk Assessments, manuals and noise information are missing. John Phyfer to organise the necessary teams to complete urgently (by end January 2011) 
  • The air machine chuck
  • The face clamp
  • The bar jack and machine clamp
  • Wray Shaw’s Platform at Kopanang
  • The Hydraulic and Air Mamba OEM’s risk assessment
  • MACHINE Operators manual (Air and Hydraulic Mamba). Servicing and parts information necessary.
  • The noise levels on the hydraulic Mamba need to be determined.
